Adapting the Parent Connector program for caregivers of adults with SMI: the Family Connector experience

Caregivers play an essential role in the recovery of their adult loved ones with serious mental illness (SMI). Promoting caregiver empowerment and well-being is critical but has often been overlooked. Family peer interventions are well-suited to fill this need; however, research is limited supporting their use with caregivers of adults with SMI. Our team is currently engaged in a study evaluating an adapted version of the evidenced-based Parent Connectors program1,2, renamed Family Connectors (FC), for caregivers of young adults recently discharged from the First Episode Psychosis program in New York (OnTrackNY). This paper, written collaboratively with family peer professionals, describes the adaptation process we followed to systematically document the family peer professional experience one year into delivering the FC program to this novel population. Modifications made to improve intervention fit included tailoring the content of the intervention, incorporating an outside approach into the program, lengthening the service in some cases, and adding a weekly peer supervision meeting. We hope that these insights are used to inform future efforts to advance support for families of loved ones with SMI, and more broadly, that our approach serves as a model for effective collaboration to improve the application of peer support services for unique groups.

Mental health care needs of caregivers of people with Alzheimer’s disease from online forum analysis

Informal caregivers of people with Alzheimer’s disease and related dementias (ADRD) are at risk of poor mental health. This study aimed to investigate the feasibility and validity of studying caregivers’ mental stressors using online caregiving forum data (March 2018–February 2022) and natural language processing and machine learning (NLP/ML). NLP/ML topic modeling generated eight prominent topics, which we compared with qualitatively defined themes and the existing caregiving framework to assess validity. Among a total of 60,182 posts, 5848 were mental distress-related; for the ADRD patients (symptoms, medication, relocation, care duty share, diagnosis, conversation strategy) and the caregivers (caregiving burden and support). While we observed novel topics from NLP/ML-defined topics, mostly those were aligned with the existing framework. For feasibility assessment, qualitative title screening was done. The findings shed new light on the potential of NLP/ML text analysis of the online forum for informal caregivers to prepare tailored support for this vulnerable population.

Empowering Alzheimer’s caregivers with conversational AI: a novel approach for enhanced communication and personalized support

Alzheimer’s disease and related dementias (ADRD) place a significant burden on caregivers. To address this, we developed ADQueryAid, a conversational AI system designed to empower ADRD caregivers. Built on a Large Language Model and enriched with ADRD knowledge, ADQueryAid uses Retrieval-Augmented Generation to provide personalized and informative support. A user study comparing ADQueryAid to a baseline model (ChatGPT 3.5) demonstrated its superior usability, offering contextually relevant information and emotional support. This study highlights the potential of tailored AI systems to enhance the caregiving experience, paving the way for future research on their long-term impact.

The salivary metabolome of children and parental caregivers in a large-scale family environment study

Human metabolism is complex, and is impacted by genetics, cohabitation, diet, health, and environmental inputs. As such, we applied untargeted LC-MS metabolomics to 1425 saliva samples from a diverse group of elementary school-aged children and their caregivers collected during the Family Life Project, of which 1344 were paired into caregiver/child dyads. We compared metabolomes within and between homes, performed population-wide “metabotype” analyses, and measured associations between metabolites and salivary biomeasures of inflammation, antioxidant potential, environmental tobacco smoke (ETS) exposure, metabolic regulation, and heavy metals. Children and caregivers had similar salivary metabolomes, and dyad explained most metabolomic variation. Our data clustered into two groups, indicating that “metabotypes” exist across large populations. Lastly, several metabolites—putative oxidative damage-associated or pathological markers—were correlated with the above-mentioned salivary biomeasures and heavy metals. Implications of the family environment’s effects on metabolomic variation at population, dyadic, and individual levels for human health are discussed.

Symptomatic associations and sexual differences in depression and communication

Previous studies have explored the associations between parental and offspring’s depression and parent-child communication. However, few studies have investigated their symptomatic associations and potential sex differences. Therefore, this study aims to examine their associations and sex differences in parents and offspring. Based on the China Family Panel Studies (CFPS)-2020 study, depressive symptoms and parent-child communication were measured by the 8-item Center for Epidemiologic Studies Depression Scale (CESD-8) and independent questions, respectively. Network analysis was used to investigate the associations and to compare the sex differences of parents and offspring. A total of 1710 adolescents were included after cleaning process (N = 28,530). There were significantly stronger associations in boys’ “anhedonia” and “arguments with parents”, and in girls’ “happiness” and parents’ “joyfulness”. Furthermore, there were same-sex depression associations between children and parents (e.g., boys’ “despair”–fathers’ “joyfulness”; girls’ “anhedonia”–mothers’ “loneliness”). These results would help us to better understand the in depression and communication nuanced associations and to develop effective strategies for improving parental and offspring’s mental health.
