Trends in low global warming potential inhaler prescribing: A UK-wide cohort comparison from 2018–2024

Methods

Primary care data for low and high GWP inhalers prescribed within the periods under consideration were collected monthly in all four nations using a standard basket of low GWP inhalers. A source of inhaler type e.g. DPI, MDI, SMI and carbon footprint values for individual inhalers was identified from PrescQIPP11. Data was collected in Wales from CASPA (Comparative Analysis System for Prescribing Audit) by All Wales Therapeutics and Toxicology Centre (AWTTC), in England from ePACT2 by AWTTC, in Scotland from the prescribing information system (PIS) by Paul Paxton and from Northern Ireland by the Family Practitioner Services (FPS) Directorate of the Business Services Organisation. Data on the carbon footprint in Wales from inhaler prescribing was calculated by then multiplying the assigned carbon footprint of an inhaler by the number of items within the dataset. Where a short acting B2 agonist (SABA) was prescribed generically, it was assumed that it was high GWP salbutamol.

The primary outcome was the change in the percentage of low GWP inhalers by country from 2018–2024.To identify any statistically significant changes in the linear trends of percent low GWP inhalers we used Joinpoint regression analysis software12. Joinpoint identifies the best fitting model from the range of joinpoint models evaluated. All of the models assumed constant variance with first order autocorrelation estimated from the data and the maximum number of possible joinpoints constrained to 3. All other settings were as per the system defaults with the significance level of 0.05. The data were exported to Stata13 for production of the plots. Joinpoint regression models were also used to identify the best fitting regression lines for the actual number of prescriptions of each inhaler type. The average daily prescription counts were based on working days and therefore took account of weekends and bank holidays.

Results

The best fitting Joinpoint models for three nations had two distinct trends, with changes occurring in December 2021 for England, March 2022 for Wales, and December 2022 for Scotland (Fig. 1). England, Wales and Scotland ended on an upward trend, but only Wales had a higher percentage of low GWP inhalers at the end of the study period than at the start of it.

The rates of change were significant for all trends for all nations except Scotland after December 2022 with the greatest rate of change being in Wales from March 2022 (Fig. 1 & Supplementary Table 1.1)

When the trends for the average prescriptions for high and low GWP inhalers were compared between the countries (Fig. 2 & Supplementary Tables 2.1, 2.2), only in Wales was there a simultaneous reduction in high GWP inhalers and increase in low GWP inhalers. In March 2020 there was a notable, but temporary increase in high GWP inhalers corresponding to the onset of the COVID pandemic in all four countries. In Wales, this was accounted for predominantly by a large increase in the prescription of short acting B2 agonist (SABA) MDIs, with much smaller increases in inhaled corticosteroid (ICS) MDI, ICS DPI and inhaled corticosteroid/long acting B2 agonist (ICS/LABA) DPI (AWTTC data).

When comparing prescribing data from September 2018 to August 2019 with September 2023 to August 2024 in Wales, there was a reduction in the carbon footprint from inhalers of 20,303 tonnes of CO2 equivalent.

The proportions of low GWP inhalers were reducing in England, Wales and Scotland until various points in 2021/22 when the trends reversed. The proportion of low GWP inhalers in Northern Ireland declined across the whole period under study. It is notable that a national incentivised quality improvement project to switch inhalers to low GWP devices ran in England from October 2021 until January 2022 and again from April 2022 until March 2023, and in Wales from October 2022 until March 2024, but did not exist in Scotland or Northern Ireland. In addition, greener practice groups launched an open access website in April 202214 that highlighted the green agenda in inhaler prescribing amongst other environmental issues. However, the change in Wales started before these two interventions and has been greater than in England. Wales implemented a national respiratory toolkit across the whole of primary care promoting the green agenda. This toolkit included updated guidelines denoting inhaler carbon footprint delivered in digital and print format to every practice in Wales from the autumn of 2021. In addition, a coordinated campaign was initiated in the same period including webinars and video-based updates delivered to 13,000 healthcare professionals (HCP) on the digital platform and 18,200 respiratory app users with the aim of motivating both patients and HCP to switch to low GWP inhalers (Supplementary Table 3). The lack of change in England from either the greener practice site or the incentivised QI project and the timing of implementation of the respiratory toolkit in Wales suggests that it was the latter that was the most important factor in creating behaviour change. The limited effect of the incentivised QI project is unsurprising since the evidence that financial incentives improve outcomes is weak15,16. We have noted a significant reduction in the carbon footprint from our inhaler switching of 20,303 tonnes in Wales. Whilst this is a small step in contributing to combatting the climate emergency, the wider societal cost of carbon has recently been calculated to $185 (or £145) per tonne of carbon dioxide17. By this analysis, we have saved £2.9 million by our actions in Wales.

There are a number of limitations. We have reported only on primary care prescribing data, whilst being aware that this represents the vast majority of all inhaler prescribing. We cannot be sure of the relative contributions of different influences in creating change, although we note the lack of behaviour change in the other UK countries where various factors were at play. We have not measured changes to moderate GWP short acting B2 agonist inhalers (Salamol from salbutamol/Ventolin) which may have been important in reducing the carbon footprint of inhalers in all countries. We also recognise that future generations of propellants for MDI will have lower GWP which will change the dynamic of inhaler prescribing. It is clear that additional factors to national guideline policies such as the toolkit approach adopted in Wales are necessary to create widespread behaviour change in inhaler prescribing.

Over-the-counter short-acting β2-agonist purchase and asthma-related health outcomes: a post hoc analysis of the SABINA III study

This post-hoc analysis of the SABINA III study evaluated the association of short-acting β2-agonist (SABA) prescriptions and self-reported over-the-counter (OTC) SABA purchase in the previous 12 months with asthma-related outcomes using multivariable regression models in 4556 patients (mean age, 48.9 years). Of the 2810 patients prescribed ≥3 SABA canisters, 776 (27.6%) also purchased ≥1 SABA OTC. This subset of 776 patients reported the highest disease burden; 73.2% had ≥1 severe exacerbation and 55.7% had uncontrolled asthma. Asthma-related outcomes worsened with any SABA OTC purchase, regardless of SABA prescriptions; disease burden was the highest in patients with ≥3 SABA prescriptions and ≥1 SABA OTC purchase vs 1–2 SABA prescriptions only (86% lower odds of having at least partly controlled asthma and 124% increased incidence of severe asthma (both P < 0.001). These findings emphasize the need to implement policy changes to restrict SABA purchase without prescriptions and ensure access to affordable asthma care.

The Reliever Reliance Test: evaluating a new tool to address SABA over-reliance

Over-use of SABA is associated with poor asthma control and greater risk of exacerbations and death. Identifying and addressing the beliefs driving SABA over-reliance is key to reducing over-use. This study aimed to assess the utility, impact and acceptability of the Reliever Reliance Test (RRT), a brief patient self-test behaviour-change tool to identify and address SABA over-reliance. Patients with asthma who completed the RRT in Argentina were invited to an online survey exploring the acceptability of the RRT, and its impact on patients’ perceptions of SABA and intention to discuss asthma treatment with a doctor. 93 patients completed the questionnaire. The RRT classified 76/93 (82%) as medium-to-high risk of SABA over-reliance (a mindset where SABA is perceived as the most important aspect of asthma treatment), with 73% of these reporting SABA overuse (3 or more times a week). 75% intended to follow the RRT recommendations to review their asthma treatment with their doctor. The RRT is acceptable to patients and was effective at raising awareness of, identifying and addressing SABA over-reliance and encouraging patients to review their treatment with their doctor.

Uncovering patterns of inhaler technique and reliever use: the value of objective, personalized data from a digital inhaler

Electronic inhalers provide information about patterns of routine inhaler use. During a 12-week study, 360 asthma patients using albuterol Digihaler generated 53,083 inhaler events that were retrospectively analyzed. A total of 41,528 (78%) of the recorded inhalation events were suitable for flow analysis (having a PIF ≥ 18 L/min and <120 L/min). Median PIF, inhalation volume, inhalation duration, and time to PIF for these events steadily decreased between the first and last 10 days of the study, by 5.1%, 12.6%, 15.9%, and 6.4%, respectively. Continuous short-acting beta2-agonist (SABA) overuse, defined as ≥2 SABA inhalations/week throughout the study period, was seen in 29% (n = 104) of patients. Of 260 patients with ≥1 instance of acute short-term SABA overuse, 55 (21%) had a confirmed exacerbation. Electronic recording of real-life inhaler use can capture valuable, objective information that could inform disease management and clinical decision-making.

Mulched drip irrigation: a promising practice for sustainable agriculture in China’s arid region

The long-term practice of mulched drip irrigation (MDI) has significantly advanced cotton production in China’s arid regions. In this study, we conducted an evaluation of the changes to key soil environmental factors within cotton fields under MDI, providing a holistic assessment of MDI sustainability. Specifically, long-term MDI demonstrated numerous benefits including improved soil salinity management, nutrient retention, soil structure, and microbial functions. Collectively, these changes contribute to enhanced overall soil quality. Our research also confirms MDI as an effective integrated water-saving technology approach. Affirming the sustainability of widespread MDI promotion, our findings firmly support its potential as a sustainable and viable solution for oasis agriculture in arid lands, conferring long-term benefits to agricultural production in these regions.

Asthma prescribing trends, inhaler adherence and outcomes: a Real-World Data analysis of a multi-ethnic Asian Asthma population

Inhaled corticosteroid (ICS) is the mainstay therapy for asthma, but general adherence is low. There is a paucity of real-world inhaler prescribing and adherence data from Asia and at the population level. To address these gaps, we performed a real-world data analysis of inhaler prescribing pattern and adherence in a multi-ethnic Asian asthma cohort and evaluated the association with asthma outcomes. We performed a retrospective analysis of adult asthma patients (aged ≥18 years) treated in the primary and specialist care settings in Singapore between 2015 to 2019. Medication adherence was measured using the medication possession ratio (MPR), and categorised into good adherence (MPR 0.75-1.2), poor adherence (MPR 0.75) or medication oversupply (MPR > 1.2). All statistical analyses were performed using R Studio. 8023 patients, mean age 57 years, were evaluated between 2015 and 2019. Most patients were receiving primary care (70.4%) and on GINA step 1–3 therapies (78.2%). ICS-long-acting beta-2 agonist (ICS-LABA) users increased over the years especially in the primary care, from 33% to 52%. Correspondingly, inpatient admission and ED visit rates decreased over the years. Between 2015 and 2019, the proportion of patients with poor adherence decreased from 12.8% to 10.5% (for ICS) and from 30.0% to 26.8% (for ICS-LABA) respectively. Factors associated with poor adherence included minority ethnic groups (Odds ratio of MPR 0.75–1.2: 0.73–0.93; compared to Chinese), presence of COPD (OR 0.75, 95% CI 0.59–0.96) and GINA step 4 treatment ladder (OR 0.71, 95% CI 0.61–0.85). Factors associated with good adherence were male gender (OR 1.14, 95% CI 1.01–1.28), single site of care (OR 1.22 for primary care and OR 1.76 for specialist care), GINA step 2 treatment ladder (OR 1.28, 95% CI 1.08–1.50). Good adherence was also associated with less frequent inpatient admission (OR 0.91, 95% CI 0.84–0.98), greater SABA overdispensing (OR 1.66, 95% CI 1.47–1.87) and oral corticosteroids use (OR 1.10, 95% CI 1.05–1.14). Inhaled corticosteroid (ICS) adherence has improved generally, however, poor adherence was observed for patients receiving asthma care in both primary and specialist care, and those from the minority ethnicities.
